Cancellation Policy Middlewich Street Dental

  • If you need to cancel your appointment we require 48 hours’ notice. We ask for all cancellations to be made in writing over email, in person or via our phone line, a voice message should the practice be closed is also acceptable.
  • For any private appointments giving less than 48 hours’ notice will result in a minimum charge of £30, although this can be higher depending on the complexity of the procedure which would
    have been already prepared for. 
  • For any patient wishing to dispute the policy then please put it in writing to the deputy practice manager: [email protected]

All patients of Middlewich Street Dental Practice are required to read our terms and conditions which form part of our cancellation policy.

Our terms and conditions state:

  1. 48 Hours’ notice is required to cancel any appointment at Middlewich Street Dental Practice
  2. Patients are to pay all fees when they become due
  3. If an FTA occurs a minimum cancellation fee of £30 will be charged (Private appointments only).

Cancelling on behalf of someone else
Please note that we keep strictly to the rules of clinical confidentiality. If you are cancelling on behalf of someone else, we have to know that you have their permission to do so, unless they are incapable (because of physical and mental illness) or you are the parent/family member or carer for the patient.

Cancelling due to illness

Patients who are unable to make their appointment due to illness must, where possible, contact the practice 48 hours before the appointment if they cannot attend. Failure to notify us may result in a failure to attend charge unless you have exceptional circumstances . We may at times ask for proof of this .We Thank you for your understanding.

Booking appointments

  • If you require an appointment lasting longer than 30 mins then a deposit will be required upfront prior to booking the appointment.
  • For patients booking complex treatments, a deposit will be required prior to booking your appointment. If you fail to provide 48 hours’ notice to cancel your appointment, then you may lose this deposit as we would have already prepared for the procedure.
  • All new patients are required to pay the fees for the new patient exams or emergency appointments prior to booking their appointment
  • Patients are to pay all fees when they are due
