Cancellation Policy Middlewich Street Dental

Covering Short Notice cancellations and Fail to Attend

  • If you need to cancel your appointment, we require two working days’ notice. We ask that all cancellations be made in writing over email, in person or via our phone; a voice message should the practice be closed is also acceptable.
  • For any private appointments, giving less than two working days’ notice will result in a minimum charge of £30, although this can be higher depending on the complexity of the procedure, which would
    have been already prepared for.
  • For any patient wishing to dispute the policy, then please put it in writing to the deputy practice manager, [email protected]

All Middlewich Street Dental Practice patients must read our terms and conditions, which form part of our cancellation policy.

Our terms and conditions state:

  1. Two working days’ notice is required to cancel any appointment at Middlewich Street Dental Practice
  2. Patients are to pay all fees when they become due
  3. If an fail to attend or short notice cancellation occurs, a minimum cancellation fee of £30 will be charged.

Cancelling on behalf of someone else

Please note that we keep strictly to the rules of clinical confidentiality. Suppose you are cancelling on behalf of someone else. In that case, we have to know that you have their permission to do so unless they are incapable (because of physical and mental illness) or you are the patient’s parent/family member or carer.

Cancelling due to illness

Patients who cannot make their appointment due to illness must, where possible, contact the practice two working days before if they cannot attend. Failure to notify us may lead in fee, for any exceptional circumstances please discuss with our practice manager. We may, at times, ask for proof. We thank you for your understanding.
